Create Fillable Word Table

I have a Word table that needs to be made fillable. It's one side of an 8 x 11 1/2 page.

Requirements:
- Compatibility with Microsoft Word
- Input fields: text fields and number fields
- No predefined options needed for any fields

Ideal Skills & Experience:
- Proficient in Microsoft Word
- Experience creating fillable forms and tables
- Knows how to make a spreadsheet look like my word document
