Convert word document to an online fillable document with electronic signature

I have a word document with tests that i print. I then check each tests on an offline machine I have setup, if the test passes i tick 2 boxes to say it passed and write in my initials and date to specify the person who carried out the test and the date it was done. At the end of the document myself and some other people sign the document. I want to try and automate this so i dont need to print it and i can fill in my initials and date for each test electronically. The tests in this word document changes all the time so it will need to accomade this.
