Convert Word document to interactive client survey

We help clients design houses and we have an information sheet they need to fill out prior to starting their project. I need someone to take a Word document and convert it to an interactive client information sheet that has tick boxes, write in boxes and drop down boxes that clients can input information and answer questions within the document. Then they can save and email it back to us. I have enclosed the Word document that I need converted.
