Convert PDF to word document

I have a PDF of a hand book that I need converted to a word document. the word document needs to be properly formatted with the correct headers, sub headers, table of contents.
I understand it is easy enough to export from ADOBE, but it doesn't work correctly. I need any spacing issues corrected, headers and footers applied.
