Convert PDF Document into a Microsoft Word Document

Please convert the PDF document into an editable Microsoft Word document. The layout must match the PDF. The Word document must be editable and it must allow the form to be filled out by a user via Microsoft Word.
The Word documents must:
- Use font Helvetica. Size 10pt preferred
- All text must be exactly the same as the text the PDF provided. Do not omit any words or punctuation
- Have similar page layouts as the PDF provided
- Be editable
- Checkboxes active
- Provide 1 Word document upon completion
- Image at the top is not required
