I require efficient and expert assistance in the creation of a Word document incorporating various form fields: Text fields, checkboxes, and dropdown menus.
The arrangement of these form fields will not follow a conventional single or multiple column layout. Instead, I require a customized arrangement modeled on a specific example I already have.

It will be good, if there a single page, that shows all these yellow text as form fields to fill in. Then Click Generate, it generates the word doc based on the filled in values.
Some text may have to be removed based on checkbox selections in the form.
