In Wix, attached child information to an application form

I have a Wix website, parent can login and create child information and application form. I want to allow a parent to attach the child information to an application form in the UI.

Please let me know how you can handle it. What information do you need
How much effort in hours it will take.
I would expect that it is a small project.
