Wix Maintenance Form to Jotform

I need my existing Maintenance Request form, currently running on Wix at https://www.pilotbodycorp.com.au/oc-maintenance-request, rebuilt in Jotform. Every field you see there must appear exactly the same in the new version, with matching order, labels, required-field rules and conditional logic (if any). The look and feel—spacing, colours, section headings and overall 1-page layout—should remain unchanged so visitors experience a seamless transition.

No extra integrations, scripts or third-party add-ons are necessary; I simply want a faithful Jotform replica that I can embed on our Wix “Forms” page alongside the others (example: https://www.pilotbodycorp.com.au/forms).

Deliverables:
• A working Jotform link in my Jotform account (or transferred to me) containing all original fields
• Embed code or Wix iframe snippet tested and confirmed to load correctly on a Wix page
• Short hand-over note outlining where to adjust styles or field settings in the future

Once delivered, I will drop the code into Wix and do a final visual check, so please keep to the existing design pixel-for-pixel as much as Jotform allows.
