Wanted: WIX Wizard to build online forms and data management on existing site

Swim Safe Forever is a small charity that is going to end infant drowning. We need out 3 to 4 intake forms brought online so we can scale.
Forms are basic will require the upload of documents with the forms and the ability to see and share add and edit data as it is accumulated and update throughout its lifetime.
We are trying to avoid touching, copying, and printing documents more than necessary.
