Embed Donation Form on Wix - U.S. BASED ONLY

I need a donation form embedded on our Wix Studio donation page for our small nonprofit organization. Web donation page and auto thank you page exists, as well as stripe account ready to go per stripe

Requirements:
- Both single and recurring donation options - default recurring w suggested amts
- Integration with Stripe as the payment processor
- Fields for:
- Donor's name, contact info (email, phone)
- Tribute - toggle)

Form mocked up, stripe account ready to go- needs to be designed but content all there

Required Skills & Experience:
- Deep experience with Wix Studio + embedding/integrating Stripe forms
- Knowledge setting up donation forms with customizable fields
- End to end testing
- Customer portal to own domain
