Database work for Wix forms

Hello there,
We have created two forms on Wix. On one form, the person will enter details like country, location, topic, etc and this needs to be saved in a database format which can be retrieved as the options for the other form, where another person will select details/slots from this created database where he will select options like country, location, topic, etc.
Once the filled amount of slots is finished, it should not show in the second form for selection. The UX for these forms is made already. I am sharing how the second form looks tentatively.
We also will require support from the freelancer if any changes are needed in the form/database for one month.
