Create a Windows .exe that counts all media files in a folder and its subfolders and creates/updates a CSV with the counts.

I have a large video archive. In my MAIN folder I have hundreds of folders with the Channel's names. Inside of those folders videos and audio files and text files and more subfolders with videos and audio files and text files. I have millions of media files. Millions.
REQUIREMENTS OF THE PROJECT:
(1) I'm running Windows 10. I need a little program that will execute in the root of the directory and count all media files only (MP4, MKV, MP3, .OGG, .AAC, WEBM, WEBV, etc...) in each folder and its subfolders. Then post that count in a CSV text file. Every time I run the program it needs to update the CSV file with the date it counted the files.
(2) I'd like a column that i can write comments into so that will not be changed when the program updates the CSV file.
(3) Do NOT count other file types like .TXT, .DESCRIPTION, .JPG, etc...
(4) The Program should be called "File Count Journal"
