JOB ANALYSIS SUITABILITY

We wish to measure the suitability of the job.
There are questions that candidate have to answer and from the given answer, the system will calculate their readiness for employability.
All the entered data by each user will be stored in a database. The user then can see their result/rpt/summary which is auto calculated (using the Cronbach Alpha formula).
Meaning there will be simple user registration page that can record the user where they can come back later to see their result and answer the questionnaire again which will be stored as a second attempt and so on.
Admin should be able to see all the entries, generated report and delete selected entries.
Sample of questions are attached here. There are 9 pages of questions which pg 3-9 are all likert scale question (option 1-5).
I need all the source code for further modification later if necessary.
