printing layout problem

As part of a graphic project for a medical institution.
I have successfully completed a questionnaire before an exam that will be addressed to all patients coming to do an MRI.
I would need to print this document in color but it turns out that I have a problem of printing there is a lost background that instale on the four corners of the form what comes cuts the design and I can not correct this in the parameters.
So I would need help to correct that.
