build a report form that is easy to input images and has bullet point information for each image

I am looking for someone to create a report form for me that is easy to input images into and also contains bullet point information for each image. The reports are used to guide home owners on things that need to be done before they put their house on the market. Attached is the template I currently used. Its ugly and time consuming to adjust for each new house. The report form should be built on Google Docs and the layout must follow my rough idea. I do not require any additional features such as automated calculations or data validation. The freelancer must be able to discuss the project in detail and provide updates throughout the process.
