Website to send an order form

I need a very simple website (Or an APP) that allows to my colleagues to compile a reservation form for a airport transfer and send to a specific mail.
Each user must have a own account and each user must have a the possibility to print a report every month.
Who receive the mail of reservation must receive the mail from only one email address.
