Website for online clinic (landing page, intake forms, payment, emails)

I am looking for a talented web developer to design and build a website for my online clinic. The main goal of the website will be to provide telemedicine services. Scope in the first phase will include landing pages, patient intake forms, payment integration, and email templates.

Subsequent phases would include the addition of medication delivery, and the eventual goal would be to shift to a SAAS business model, offering the same functionality to other clinics. Seeking a quote for the first phase of the project to begin, but interested in building a long-term relationship with a web developer who has B2B SAAS experience and could continue evolving the platform over time.
