- I need to create a web based survey form which contains Agent name, Application Number (auto generate), Name, Contact Number, Address, and GPS location of the address.
- The form must be able to record Offline as during the survey, the access for internet might not be available.
- The application form must then able to print and save in PDF.
- The reporting must be able to be projected via Google Earth (of all application received).
- The survey must be user friendly on Desktop, Mobile and Tab.
