Simple Website form that is opened when QR code is scanned

Main Objective of Website: We want to create a simple website form, where mobile users will scan a QR code (through their camera app) > (open website link) and fill in a simple form. This is just a prototype app and is not meant to be big.

Goal of the app: This project will help us with pest control. When people go for a walk in the park and see a rat or squirrel, people can report this sighting via a QR code that is located somewhere in the park. This information is then captured on a simple database so that the pest control team can know how many sightings there have been.

Note: this is a very simple website form. We need two QR codes, a simple form, and a simple database where the information is captured.

Scenario: when going for a walk in the park, if someone sees a small animal, like a rat or squirrel. People will be able to scan a QR code at the entrance of the park. After scanning the QR Code, a link to a simple web form will pop up, people will click the link and people will input some information on the form. Like the type of animal, how many, date and time of the sighting, and lastly press a submit button which will capture this information in a simple database.

Note: The location of sightings is predetermined via the QR code. We need two QR codes, one for PARK A, and one for PARK B.

Breakdown of what we need:
2 x QR codes, one in PARK A, and one in PARK B.
1 x simple web form - please see attached file for the specification.
1 x database - that captures this information.
