Simple WEB app Responsive Design for mobile devices

I am looking for an experienced web application developer to as a start make a simple app including a map view displaying objects and information about the objects.
It will be a simple functionality but the importance is to use responsive design so it can be used on different mobile devices and different browsers.
For this first step I will propose a fixed price project. If it works fine the project may be extended and based on a hourly rate.

We need to use modern style and commonly use web framework. Even though it may initially be a simple web app it must be high quality work and user experience must be high. If this initial step goes well it will be more work to do.

Please see attached document with brief information.. The first step I want the freelancer to ask the right question in order to provide a fixed price. I also want to see example of work done so we can see that the user interface design is modern and user friendly. For freelancers that will be considered I will provide more information.
