Shopify landing page

Build a Shopify landing page to collect new customer name, email address and mailing address (US address only). Upon completion, present the customer with a thank you page. A free item will be mailed to the customer. A customer (or mailing address) may only get this free offer once. See the attached image for an example, except this must include mailing address.
