UI Required: Need couple of forms, report and a tracker page

Hi,
I am currently looking for UI developer for very simple project. He/she need to create a UI which contain couple of pages.
1) Resource Page
2) Environment Page
3) Report Page
4) Tracker Page
Resource Page - Will be a form containing some textbox, dropdown, checkbox. Once user fill that information it will go to summary page and if everything looks good. Will submit the request.
Environment Page - Again this will be a form where the user can add, edit and delete environment.
Report Page - The report page will contain a couple of visualizations about the user-created resources.
Tracker Page - Will contain job which user submitted for creating the resource.
OKTA Integration - At the end the UI need to integrate with OKTA
Developers need to develop only UI. No need for backed development.
