Pop Up Form & Landing Page

I am looking for a skilled freelancer to create a Pop Up Form and Landing Page for my website. The main goal of this project is to collect email addresses from users who visit my site. The design style that I prefer for the form and page is Corporate and Professional. I would like the form to collect the user's name and email address. Ideal candidates for this project should have experience in web design and development, as well as expertise in creating effective pop up forms and landing pages.

I want the pop up form to be appeared in the middle of each post. So I need a landing page and pop up form together with exit intent also. I have a wordpress site with a beehiiv account. Using mailchimp now, but want to remove it.
