Need a form for Wordpress site

Currently the webform I have is made on a LARAVA platform. Please check out justsayuncle.ng and scroll to where to become a Contestant titled ‘Register Here’. Try and register and see where it prematurely stops.
What I need:
- The new website is being developed in Wordpress
- The form will need to be changed to a Wordpress form unless you can fix the Larval and add it to the new Wordpress site
