Minimalist Checkbox Score Page

I need a simple, minimalist design page added to my website. The page will feature 10 checkboxes related to a specific phrase. Each checkbox corresponds to a phrase that carries a certain number of points. Users would be checking the phrases applicable to themselves. Which in turn will be giving the total score with a congratulatory message in other page.
