MVC Developer accessing SQL database

I’m an IT data professional with over 40 years experience. I have created a database system that needs a web interface. I have started writing this in MVC but my web skills are slowing my progress down so I’m looking for help with this side of my project. I’ve attached one very basic page of the required website and I want to ascertain whether the time, cost and communication works for both of us. I don’t want to over complicate the specification side but at the same time, I recognize the need for accuracy. I’d like you assess the attachment and see if you can work with this format or what you would expect in addition. How much time and money something this will take to develop. As I say, this is only a simple example as I think this is a good place to start. If we agree to go ahead, I will provide access to the database in Azure where I expect the website to be published too. I thank you for your time and look forward to your response.
