I'm looking for a skilled individual to create a HubSpot CMS page complete with a form. The purpose of this form is to collect customer information, including their Name, Email Address, Phone Number, Address, and Date of Birth.
The form must be smartly designed and user-friendly, ensuring that visitors to the page are encouraged to submit their details.
The form should create a new lead in HubSpot.
The page should use the header and footer of my existing pages.
