MD5 Hash Generator Connected to a Survey

I am looking for a skilled developer to create an MD5 Hash Generator that will be connected to a survey. The purpose of this generator is to encrypt survey responses for data privacy. The level of encryption security needed is basic. The MD5 Hash Generator will be handling Personal Identifiable Information (PII) data.
I am conducting a survey that needs to be anonymized while also tracking an individual anonymous user through multiple administrations over time.
Thus, I need to create a webpage in which the first item is a MD5 Hash Generator (https://www.md5hashgenerator.com/).
The page should ask the person to enter their First Name, Last Name and Birth Date (DD/MM/YYY). Example: “Anna, Rejs, 22/10/2001”
Then the person should hit “Next” (the MD5 Hash should be created - example: 6e022294bfc9bc54969e8e3f3e997727)
and the MD5 Hash should be automatically entered into the first question response of this survey:

The user should then be able to go on and complete the survey.
