From PDF to HTML - Emails

Hi!
I need to convert this attached pdf, to an html page.
The result will be used as an HTML signature file on Mozilla Thundirbird, so..
- We cannot have external css (only one file html, in style css)
- We cannot use an images folder: images will be linked from ftp space (temporarily please use your ftp support space)
Please, use clear code (not automatic conversion from PSD or similar).. we will need to change text in future.. so.. good formatted html ;)
