Customer Enters a serial number, website gives a result

I have a sports card website. We certify our cards with a serial number. I use shopify. I can create a special page. I need the code to have a field on the page, that a customer can enter their number for their sports card ex. 3014 and hit enter and it will give them a result "1927 Topps Babe Ruth Card #37, Graded 9/10"
I would need to have a way to update the data with a spread sheet. Trading card companies have these pages if you need to see an example.
