Build a New Website

Background: I would like to create a webpage for an optometry practice that is focused on specialty care (specialty contact lens and myopia management specialties). The website will be initially informational. I would like to have the flexibility to sell products on the website in the future.

Requirements:
• A clean and modern design that is aesthetically pleasing.
• I would like the website designer to have experience in graphic design/logo design skills, marketing, and
SEO.
• The website will need to be a responsive web design
• Unlimited revisions
• Will need a pages that function similar to this:

• Will need a page to integrate with electronic medical record scheduling and payments

Pages:
• ~10-15 pages

Budget:
• $400, but will pay more for the right fit; After completion of website, would pay hourly rate for any additions/edits after a specified period of time.
