Build Us A Medical Appointment Website
Budget: $750 – $1,500 USD
Looking to assemble a basic Medical Appointment Booking Website. Need a competent party to design and build the user interface as well as the back-end.
Basic requirements for functionality include:
1) Ability to Schedule their own appointment
2) At Least a day before the scheduled day they would be informed of a little background and face of the examiner they would be conducting the examination with.
3) Also on the website I would need the agent to have the opportunity to fill out a new request form for each client so we can be able to plug into our system so we can know what is needed for each examination. For example, Name, DOB, Policy #, Policy Amount, Requirements. (Requirements meaning if they need blood taken, urine, measurements, EKG)
Each examiner will cover a certain location at a specific time. Which would give the clients and agents and opportunity to choose what day and time work best for their schedule
