We need to fully utilize Adobe Pro on the web

We are a golf association. We are trying to put a web form on our GoDaddy site. (see attached) The players open the form and fill in info. Thias works pretty good, but the 'Total' cell will not add up. We are looking for an Adobe Pro consultant that can walk us through the web form set up.
