Create an interactive form

Create a short online survey form. It should be answered from any browser. The format of most answers are text or constant sum. All the results must be stored in an Excel file (accumulative input). Everytime a person answers the survey, the system sends an email. Texts should be easily modified. New questions should be easy to add. Background colors should be easy to change. Please see attached mock up (survey is in spanish).
