WordPress Signable Intake Form

I’m running a WordPress site with a booking plugin and I need every new client to complete – and digitally sign – an intake form during the booking flow. The form must collect three categories of data: personal details, service preferences, and any health or legal information that I specify. A true digital-signature field (not a simple checkbox or typed name) is required so the signed document is legally binding.

Once the form is submitted, I want the executed copy emailed directly to me for records. The guest should see the usual booking confirmation page and email from my current system; nothing else about that flow should change.

Here’s what I need delivered:
• A cleanly styled intake form that matches my site, embedded at the correct step of the booking plugin.
• Digital-signature capability using a reputable WordPress-compatible solution (e.g., WP Simple Pay Signature, Gravity Forms Signature Add-On, or a custom JavaScript/REST approach).
• Automatic email of the completed, signed form to my inbox in PDF or another tamper-proof format.
• Any settings or short-codes documented so I can adjust questions later without touching code.

If additional plugins are required, please note them up front so I can license or approve them. I’m ready to give admin access and test immediately once you have a staging setup.
