Sunday School Registration Form & Payments
Budget: $250 – $750 USD
I need a simple, reliable registration form for our Sunday School that sits directly on our existing church website—not a pop-up or external page. The form must capture all the basics (child’s name and date of birth, parent or guardian details, multiple phone numbers, emails, emergency contacts, and any extra notes) through a mix of text fields, dropdown menus, and the occasional checkbox for permissions or media release.
Once a parent completes the form, they should be taken straight into a Square checkout flow to pay the registration fee. After payment, the form data and the Square transaction ID need to land in the same place so we have a clean record for each child.
Key goals
• Fully embed the form in our current site theme, matching fonts and colors so it feels native.
• Configure Square so the fee is automatically added, tax-exempt, and deposited to our existing Square account.
• Redirect to a thank-you page on our site once payment is confirmed, and send a confirmation email to the parent.
• Provide clear instructions so we can update fees or form wording ourselves later.
If you have experience with Square’s Web Payments SDK, Gravity Forms + Square, or a lightweight custom PHP/JavaScript solution, please mention it. I’m looking for a working form, the integration code, and brief hand-off notes so our volunteer team can maintain it going forward.
