My website needs a clean, modern feedback form dedicated to gathering Customer satisfaction data. The form should feature a 1-to-5 rating scale as the primary input, presented with an eye-catching yet unobtrusive UI that matches my existing design language.

Key points
• Single page or modal embed that drops neatly into my current HTML/CSS stack.
• Visual polish matters: responsive layout, intuitive star or slider control for the 1-to-5 scale, clear typography, and accessible colour contrast.
• Vanilla JavaScript or a lightweight framework is fine; avoid heavy libraries unless they genuinely improve UX.

Acceptance criteria
