Simple Contact Form Creation

I need a straightforward contact form integrated into my website. The form only has to capture two fields—Name and Email—then route the submission to my chosen inbox while storing a copy in a lightweight database or flat-file for backup.
Front-end validation for required fields, basic anti-spam protection (e.g., a honeypot or Google reCAPTCHA), and clear success / error messages are important so visitors know their message was sent successfully. I will provide server access and, if you prefer, the current CSS framework in use; you are free to implement it in plain HTML/CSS/JavaScript, PHP, or a small React/Vue component as long as it blends seamlessly with the existing layout.
Deliverables:
• Fully functioning contact form code
• Instructions for embedding it on additional pages
• A brief read-me explaining any dependencies or configuration steps
