Shopify Contact Form Embed

I want to add a simple contact form to an existing page on my Shopify store. The form only needs the following fields: Name, Email, Phone and Message. Default Shopify validation is enough; no extra rules or scripts required.

You’ll receive collaborator access to the theme so you can place the form directly inside the current page template or create a Liquid section that I can drop in through the editor—whichever keeps my layout intact. The form must inherit the store’s styling, work flawlessly on mobile, and send submissions to the standard Shopify notification email.

Deliverables
• Updated Liquid/HTML code embedded on the specified page
• Any small CSS tweaks necessary for consistent styling
• Brief instructions (one-page max) on how to edit or move the form in the future
