One-Time Code Subscription Form
Budget: $10 – $30 USD
I want a simple web subscription form that only accepts users who provide a unique, single-use code. The flow is straightforward:
• The page shows a contact-style form with these fields: name, email, phone number, address, and the required code field.
• When the visitor submits, the code is checked against a stored list and must be both valid and unused.
• If the code passes, the data is saved (MySQL, SQLite, or another lightweight store is fine), the code is flagged as redeemed, and the user automatically receives a confirmation email.
• Invalid or already-redeemed codes should trigger a clear error message and block submission.
I will supply the initial list of codes; an easy way to upload or append new codes later would be appreciated but isn’t mandatory. Clean, commented code, a brief read-me with setup steps, and any necessary configuration files complete the deliverable.
