Internal Passport Process Web Form
Budget: ₹2,000 – ₹5,000 INR
I need a streamlined online form that will replace our current paper-based passport processing worksheet. The form must sit on our website, render cleanly on desktop and mobile browsers, and push every submission straight into our existing SQL-based internal database, preserving the field structure we already use for employee records.
The workflow is fairly linear: staff choose the passport type, enter applicant details, upload supporting PDFs or JPEGs, and confirm with a digital signature. Real-time validation (dates, passport number format, mandatory uploads) is essential so mistakes are caught before anything is stored. Once a record is saved, an automatic status flag should be set to “Pending Review” to trigger our in-house dashboard.
Please build the form with a framework that makes future edits easy—React, Vue, or plain HTML5/JavaScript are all fine so long as the code is well-commented. Connection to the database can be through our REST API; credentials and endpoint documentation will be provided.

• Secure file-upload handling (max 10 MB per file)
• Server-side script/service that writes data to our database and returns success/fail messages
• Brief setup guide so my IT team can deploy the code on our staging and production servers
