HTML Webform Integration
Budget: $30 – $250 USD
I need an HTML webform added to my website for 4 products. The form for each product is the same except for the description and details for the different products.
Requirements:
- Include fields: Name, Email, Phone Number, Product Quantity, Color Choice, Payment Method, Address, Payment Details, Special Instructions
- Calculate total dollar amount of selected items
- Display total next to form on the same page
- Create and add minimalist custom buttons to product pages
Ideal Skills:
