Fix Forminator Field Display

My WordPress site ran flawlessly until my hosting company migrated it to a new server. Since that move, the Forminator form on the site loads but the actual input fields are invisible—labels show, no boxes, no dropdowns, nothing to interact with. No other feature or plugin is misbehaving, so the problem seems isolated to Forminator.

I need someone who can jump in, identify why the fields are no longer rendering (conflict, missing assets, MIME type change, permission issue, caching, CDN mis-link—whatever surfaced during the server switch) and restore normal form functionality without disrupting live traffic.

Deliverable:
• Fully functioning Forminator forms with all fields visible and usable on both desktop and mobile, confirmed via test submissions.

Please be comfortable working in WordPress, Forminator, PHP, JS, and typical host-side diagnostics (error logs, console, inspector). Once fixed I’d like a short note describing what you changed so I can avoid a repeat if the host tweaks things again.

The shortcode is used and calls the correct form
[forminator_form id="255"]

But the form is not displaying properly
