Enhance PHPBB3 Plugin for Forms

I need a PHPBB3 plugin modified to support multiple application forms.

Currently this only allows for one application form. However, I need multiple, each with its own logic and settings. A detailed guide and info is on the github - and can be tested by downloading PHPBB3 and installing the plugin.
