Digital Org Chart & Forms

I need a clean, hierarchical org chart that lives online—either as a standalone responsive page or embedded in our current site/app. Each node should be easy to update, expand, or rearrange without touching code every time, so a drag-and-drop or CMS-friendly back-end is ideal. Search and zoom would be a plus.

Alongside the chart I also require three connected, staff-facing forms:
• Employee information form
• Performance evaluation form
• Leave request form

All three should follow our brand style, validate input, and save to a single data store (MySQL, Airtable, Google Sheets, or a similar cloud DB—whichever you prefer, as long as export to CSV/PDF is straightforward). If possible, let the HR team trigger PDF exports of completed forms directly from the interface.

• Branded HTML/CSS forms with data storage and export capability
• Source code, install notes, and a brief video or doc that shows how to add or rearrange positions and edit form fields
