Customer Feedback Survey With CSV Export

I need to replace my current comment-style review section with a proper survey form so I can gather clearer, more actionable feedback. The form has to support three kinds of questions:
• Multiple choice
• Open-ended questions
• Rating scale

Once a customer submits the survey, I want every response stored in a way that I can instantly download as an exportable CSV file—no manual copying or formatting on my side.
