Budget: €30 – €250 EUR
For my existing WordPress site, I would like to set up a fully anonymous customer satisfaction survey in order to optimize both the user experience and internal processes.
The survey should consist exclusively of multiple-choice questions and ask our customers about their hairstyle preferences. At the end, based on their responses, they should be able to directly select the appropriate services in our booking tool.

Technically, you may use an established form plugin such as WPForms, Gravity Forms, or a lightweight custom development in PHP/JavaScript — the main requirement is that the solution integrates seamlessly into the current theme, is responsive, and meets GDPR requirements. The responses must be stored in the WordPress database and be exportable to CSV or Excel with a single click, so our team can analyze them later. A small evaluation widget in the backend (e.g., a bar chart of answer distribution) would be a bonus, but is not required.
Deliverables
• Fully functional, anonymized multiple-choice survey, embedded into an existing page or integrable via shortcode
• Clear data flow: storage in the database + export function
• Short training session or screencast so we can adjust questions and export results ourselves

2. Do you prefer a specific form plugin or are you open to developing your own? --> open to anything, as long as it's WP-compliant and easy to use
3. Do the results need to be exported regularly or only retrieved when needed? --> only retrieve
4. Are there already specific questions and answer options, or should they be defined together? --> the questions already exist
