Add Simple Volunteer Signup Form to My HTML Website

I need a volunteer registration form added to our HTML/PHP website and it should send the data directly to my Gmail inbox no database storage needed
The site was made using a HTML template
I have the exact mockup of how the form should look
It will need basic validation to make sure mandatory fields aren’t left blank
This is a straightforward job. Looking to get it live in 1-2 days!
