Add Dropdown Form Feature

I need a simple form integrated into my existing website. The only special functionality required right now is 1–2 dropdown menus; no file uploads or CAPTCHA are necessary at this stage.

Please handle everything from building the HTML / CSS layout to hooking the form up so I can easily collect the submissions (email or a lightweight backend script—whichever you feel is more efficient for a small-scale site). The finished form should blend seamlessly with our current design, be fully responsive, and work across modern browsers.

If you foresee any quick wins—like adding basic validation or making the dropdowns dynamic via a small JavaScript snippet—let me know. I’m open to straightforward improvements provided the core requirement stays focused on those dropdown menus.
