Wordpress Redesign and Lead Form Fix

I need a simple auto insurance website redesigned. Currently it is on Wix so you'll need to make it from scratch. We are looking for a clean modern design there are not many pages it is just a landing page with a few basic inner pages. The important section is the lead form needs to work well.
